A mishap came by his hand
He caused a mishap to occur because of something he did. He committed a negative act. He brought about an undesirable outcome.

גָּרַם לְכָךְ שֶׁקָּרְתָה תַּקָּלָה בִּגְלַל דָּבָר שֶׁעָשָׂה. עָשָׂה מַעֲשֶׂה שְׁלִילִי. הֵבִיא לְתוֹצָאָה לֹא טוֹבָה.
