If you studied, you probably did not review or repeat your learning, and if you reviewed it once, you likely did not review it a third time. Figuratively: You did not study thoroughly enough or did not deeply understand what you learned. An expression said about someone to imply that they apparently did not review the material enough and therefore made a mistake.
